SmartPractice® CanadaTM is proud to be the exclusive North American distributor of the allergEAZE patch test system, which includes allergens, patch test chambers and several ancillary products.

Please note that allergEAZE products require the supervision of a licensed physician:

  • U.S. physician’s license must be valid and in good standing before products are shipped.
  • Orders must be shipped to a current business address in the U.S.; shipping to residences is not permitted.

A SmartPractice® Canada Product Consultant will contact you by phone or email to confirm your order, method of payment and delivery.

Customers can also place an order directly with a SmartPractice® Canada Product Consultant, available Monday through Friday, 9am –7pm EST at 1.866.903.2671. SmartPractice® Canada currently accepts credit cards, checks and money orders from U.S.-based customers. Please note that SmartPractice® Canada reserves the right to determine pricing and payment methods, and to modify these policies without notice.

Shipping

SmartPractice® Canada will attempt, as much as possible, to anticipate potential shipment and delivery problems, but will not be held responsible for delays due to severe weather, unforeseen circumstances, or customer-based delivery problems.

allergEAZE Allergens are shipped directly to U.S.-based customers via Parcel Post.
Expedited shipping is also available for an additional charge – please ask your Product Consultant for additional information.

Customers are responsible for proper storage and handling of allergens after delivery. Refrigerated storage (2o * 8o C, or 35.6o * 46.4o F) is recommended for all allergens after shipment.

allergEAZE Patch Test Chambers and Ancillary Products are shipped via Fed Ex ground to U.S. based customers. Expedited shipping is also available for an additional charge – please ask your Product Consultant for additional information.

SmartPractice® Canada reserves the right to determine any restrictions in shipment, carrier and method of shipment, and to modify shipping and return policies without notice. For questions about shipping, delivery or returns, please contact SmartPractice® Canada at 1-866-903-2671.

Returns

In general, allergEAZE products are not returnable. However, SmartPractice® Canada will attempt as much as possible to resolve problems to the customer’s satisfaction. If products arrive damaged, or appears defective, please notify SmartPractice® Canada immediately at 1-866-903-2671.
